While each Care Connection for Children program is funded by the Virginia Department of Health, each program is locally - not centrally - managed, offering an optimally effective, community-based approach to care. Members of the staff
understand the needs of children and families they see each day, and their compassion combined with their knowledge of
the community's many health care resources provide a foundation of support for families who are often frustrated by the
perceived health care maze.
In addition to our case management and care coordination, CCC offers direct services by way of Pediatric
Specialty Clinics. These clinics include: Cardiology, Cystic Fibrosis, Maxillofacial, Neurology,
Neurodevelopmental, and Orthopedic. Cardiology and Neurology Clinics require a physician's referral seen.
Children are eligible to receive services from Care Connection for Children if they are:
The Children with Special Health Care Needs Pool of Funds provides a limited amount of money to assist Virginia's uninsured and underinsured
children with special health care needs to receive care they otherwise could not afford (such as medical equipment, physical therapy and
medication that are not covered by the child's insurance). The staff at Care Connection for Children can assist in determining eligibility for access
to Pool of Funds.
