CDC WONDER
This database provides mortality data for all states, including county-level data. To obtain diabetes mortality rates by county, one must access the "mortality-underlying cause of death" database that can be found under "Chronic Disease Topics." Then, access the file called: Mortality for 1999-2002 with ICD-10 codes. From here, state and local mortality rates can be queried. It is recommended that the box for age-adjusting rates be checked. "Diabetes mellitus" can be selected by running a keyword search.
Virginia's Atlas of Community Health
The Atlas database contains county and zip code level data for 114 population, economic, and health indicators and it can produce maps to illustrate the health of a community. The Atlas now contains an additional 32 "Adult Health Behavior" data points, that include 2004 county-level estimates of people with diabetes and many other chronic diseases. Local-level diabetes hospital discharge rates can be found under "Ambulatory Sensitive Conditions." Use of this database is free to the public.
